General Waiver &
Release of Liability

In order to attend classes & the social dance at the Doghouse,
all guests must review & agree to the following.

Please read this document carefully before agreeing to participate in dance activities organized by Two Dogs Barking, Inc (TDB). Please seek legal counsel if you have any questions regarding this waiver. This waiver releases Two Dogs Barking Inc, its instructors and agents from liability for personal injury, including injuries resulting from dancing and the risk of COVID-19 or other infection.

1. Assumption of Risk

I recognize that dance activities at The Doghouse involve physical and mental exertion and carries the potential for personal injury. I understand that the risk of injury is inherent in dance activities and am fully aware of the risks and hazards involved. I voluntarily assume and accept all risks and potential hazards involved in dance training and related activities provided by Two Dogs Barking. 

In consideration of being permitted to participate in the dance classes, dance party, or any event or program, I agree to assume full responsibility for any risks, injuries or damages which might incur as a result of attending and/or participating.

2. Waiver and Release

I waive the right to file any claim or lawsuit against Two Dogs Barking Inc., its instructors, officers, directors, agents, or representatives for any illness, injury, or damage resulting from my participation in this dance training or all related activities, including but not limited to claim of injury, damage to facility, equipment, supervision, including negligence or acts of omission by Two Dogs Barking Inc., its employees, members, officers, directors, agents, or representatives.

3. Acknowledgment of Safety Precautions

I acknowledge that I have been informed of and understand the safety measures and precautions taken by Two Dogs Barking to minimize the risk of COVID-19 infection. I agree to adhere to all studio policies and guidelines related to health and safety, including mask-wearing, social distancing, and sanitization.

4. Integrated Agreement

I have read this waiver of liability and fully understand its terms. I agree that this liability agreement supersedes and replaces all previous agreements between parties concerning this event, whether written or oral.
